On any order received on or before April 30, 2012, select the applicable discount from the choices below ...

For orders with 25 pieces or more, you may take $1.00 off the price of each item ordered.

For orders with 50 pieces or more, you may take $1.50 off the price of each item ordered.

What are you waiting for? These discounts are only valid on orders received by April 30, 2012. After that, they're gone.

To receive discounts, you may write your order with the discounted price in the price column, or just note the deduction at the bottom of the order form.
